The City of Citrus Heights is pleased to announce City Council’s approval of the Memorandum of Understanding with Sacramento County Department of Homeless Services and Housing to deliver the Community Based Navigation Program to the City. This program will provide homeless outreach and navigation services to unsheltered individuals in the City of Citrus Heights.
Sacramento County Department of Homeless Services and Housing (DHSH) will expand on its new service model used throughout unincorporated Sacramento County, with non-profit Sacramento Covered, to provide support services, emergency shelter, interim, and permanent housing. Sacramento Covered employs Community Health Workers (CHWs) to deliver services and establish a positive relationship with local community organizations, residents, and businesses in order to foster a supportive coalition for unsheltered individuals in the City.
Through this collaboration and ARPA district-directed funding allocation of $344,836 from Sacramento County District 4 Supervisor Sue Frost, funding will be directed to DHSH to support one (1) full-time CHW, client assistance support, and associated administrative costs for the City through December 2025.

“This partnership with Sacramento County and Sacramento Covered will allow us to continue our efforts in helping those experiencing homelessness in Citrus Heights. The funding provided by Supervisor Sue Frost has given this partnership the support it needed to get to work on improving the quality of life for all our residents,” said Mayor Tim Schaefer.
This partnership is an opportunity to build on the foundation the City established through the prior relationship with Sacramento Self-Help Housing, which closed in June of this year. The Community Based Navigation Program partnership with DHSH and Sacramento Covered provides for a more effective service delivery model that results in greater efficiencies and advancements in addressing this important challenge.
